The Before & After School program is now using an online registration system. If you don't have an online registration ID, you must fill out the Registration Form from the link below. Please fill out the form completely and bring to our office located in the Recreation Building (next to Old Bridge Library) or call us at 732-721-5600 ext. 2392, 2390, or 2395. If your child is going to be absent from the program, you must fill out the form and send it to the main office so our staff gets the form. You must also fill out the Emergency Closing Parent Action Plan Form and bring this to the site your child attends.
Each link will open in a new window. Simply type in the required information, print, and sign where required. Bring the Emergency Closing Form to your child's before/after school site and hand to the before/after school staff. Do not bring it to the school's office.
