THERE ARE NO PAPER APPLICATIONS AVAILABLE
Only 1 application per person/family. After submission, successful applicants will receive a confirmation number for future reference and will be able to login to their portal account for their status within 4 weeks after the application period closes. Only 125 applications will be accepted.

Paper applications are not available.
Considered applicants must be one of the following:
Low Income – Individuals/Families Elderly - Individuals/Families
Persons with Disabilities – Individuals/Families Disabled Veterans – Individuals/Families Homeless – Individuals/Families
